In consideration of waving the installation charge for High Speed Internet Service (reg. $180 By LTC,I, __________________________________________, agree to subscribe to LTC's High Speed Internet service for a minimum of 12 months. I understand that if I move or discontinue my High Speed service within twelve months, I will be required to pay one-twelfth of the $180 installation fee for each full and/or partial month that remains under this agreement.
